1.  Please email Michelle an E-file of the DFLL Faculty Evaluation Form to michelle@ntu.edu.tw.

      Please RIGHT-click the hyperlinks below for the evaluation form designed for your rank and fill out the "RESEARCH" and "SERVICE" categories. 請選擇屬於您職級的評估表格如下（按「右鍵」下載表格）。

※ You DON'T need to fill out the "Teaching Hours" and "Student Evaluation" sections. After we gather all of the data from TA Winnie and the Academic Office, we’ll email you the scores for Teaching and ask you to proofread the data for confirmation. 

2. Submit a HARDCOPY or an OFFPRINT of your academic works:

    請繳送擬受評之著作各一份。

• For assistant professors/instructors, please submit your academic works published or presented in the past 3 years.

• For full/associate professors, please submit academic works published or presented in the past 5 years.

• Works that have been reviewed in your previous evaluation or in recruitment   are NOT acceptable for faculty evaluation. 上次評估或新聘送審過的著作不得重複繳送

• For reference to the RANKING of your evaluated works, please see the "Publication Ranking Standards for DFLL Faculty Evaluation."（教師評估著作等級評定標準）

3. Please also email Michelle a list of publications you are submitting for evaluation (an electronic file is required).

    The publication list should be typed in MLA format (with detailed data such as the publisher, date, volume and page numbers, etc.)

NOTES

1. Works that have been reviewed in your previous evaluation or selected in one's Recruitment evaluation are NOT acceptable for faculty evaluation.
