Coupon Usage and Important Notes:
- This coupon is only valid at certified regional assessment veterinary hospitals (see the list of certified regional assessment veterinarians below).
- The coupon can only be used by certified regional assessment veterinarians to conduct respiratory function assessments for French Bulldogs, English Bulldogs, and Pugs. The assessment will determine if the dog has Brachycephalic Obstructive Airway Syndrome (BOAS) and give the owner a graded evaluation and recommendations.
- This coupon is not limited to personal use, but it can only be used once and will be collected by the veterinary hospital.
- This coupon cannot be resold, exchanged for cash, used as a change, or applied to other assessment items.
- This coupon does not have an expiration date.
- If this coupon is stolen or lost, it cannot be reissued.
- This coupon should be kept properly. It will be invalid if it is damaged, unrecognizable, or photocopied.
- NTU Small Animal ENT Lab reserves the right to adjust or change the methods of distributing and redeeming coupons. Any changes will be announced on the NTU Small Animal ENT and Clinical Sleep Research website.
